EuMax Physical Therapy Consulting, PLLC  will be the responsible party collecting all information necessary to perform Functional Capacity Evaluations, as well as Impairment Ratings. 

All personal data collected by Eumax Physical Therapy Consulting, PLLC will be retained for the length of the individual case and destroyed promptly once the case concludes, consistent with legal retention periods. 

Personal data collected by EuMax Physical Therapy Consulting, PLLC will be used exclusively by EuMax Physical Therapy Consulting, PLLC to conduct Functinal Capacity Evaluations and Impariment Ratings. The completed reports will be disclosed only to the authorized parties revelant to the individual case.
